Upon submission of this form, you will receive an email confirming your registration.  A second email will arrive within 48 hours with the link to the care community. Use that link to set up your login and password. You will not be active in the care community or receive updates until you do that.  If you do not receive the second email within 48 hours, check you spam or junk email folder.
